Our approach is to constantly evolve to meet the needs of the population and work in tandem with the acute care setting to help the patient remain in their home to receive their needed services and prevent unwanted hospitalizations.
The agency’s mission is to provide professional and paraprofessional services to clients in their homes, assisting them to achieve the highest level of potential in their day-to-day self-care activities. We are committed to providing high-quality, multidisciplinary care by professionals who recognize the need for a comprehensive assessment of needs from both the clients’ and professionals’ point of view.

Philosophy
Our goals and services are based on two fundamental philosophical services:
The belief in the innate worth of the aged and disabled individual and the belief that each individual, regardless of age, race, color, creed, sex, national origin or handicap(s) is entitled to maximize his or her potential as a human being and as a member of society.
It is the contention of All Seasons Home Health and Palliative Care that the aging process is a normal state in the development of an individual and that chronic disease and disability are, to some degree, a part of the process. This agency is dedicated to rehabilitating aged and disabled individuals within the confines of their residence, in order that they may maximize their contribution and fulfill their goals as a family member and member of a society with a minimum of conflict. In accomplishing this end, it is felt that the self-respect of the individual can and will be enhanced.
Our agency is also a vital member of its community. As an employer, the agency practices non-discrimination and strives to provide opportunities for personal and professional growth. Principal Purpose
The principal purpose is to provide skilled nursing and other therapeutic and paraprofessional services in the clients’ homes or in an institution. At least one of the services shall be provided directly by agency employees. The agency must comply with all applicable regulations of the United States Department of Health and Human Services vise and rules, regulations and guidelines as set forth by the state and local governments. The agency shall refuse neither service nor employment nor discriminate against any person because of age, race, color, creed, sex, national origin or handicap(s).
